Classic vs post-op lymphatic drainage
Classic vs post-op lymphatic drainage: book classic with no surgery or before it; book post-op while you heal with a garment, drains or firm areas.
By The Renu lymphatic and massage team · 5 min read
Book classic lymphatic drainage if you have not had surgery, or if you are preparing for one: at Renu, pre-op sessions are booked as a classic 60-minute session. Book post-op lymphatic drainage from the time your surgeon clears you until your tissue has settled, especially while you wear a compression garment, have drains, or feel firm areas forming. Months out, with only a little swelling left, ask your therapist whether classic sessions now fit.
When you open Jane to book lymphatic drainage at Renu, you will see two families of sessions: classic and post-op. The hands-on technique comes from the same roots. What differs is the tissue in front of the therapist, and the extra things a healing body brings with it to the table.
The quick answer by situation
Your surgical history decides the session, not how swollen you feel.
| Your situation | Book |
|---|---|
| No surgery; puffiness, heavy legs, general well-being | Classic lymphatic drainage, 30, 45 or 60 minutes |
| Surgery booked in the coming weeks | Classic 60 minutes, booked as pre-op |
| Surgery in the last few days or weeks, cleared by your surgeon | Post-op lymphatic drainage |
| Still in a compression garment, or drains in place | Post-op |
| Firm, lumpy or uneven areas forming after surgery | Post-op |
| Two or more procedures in one surgery | Post-op, multiple surgeries, 90 minutes |
| Months out, tissue soft and settled | Ask your therapist; classic may now fit |
What changes after surgery
A post-op session treats healing tissue, so the therapist is managing more than fluid. Surgery disrupts the lymphatic vessels in the operated area, which is why fluid pools, bruising spreads and firmness can develop. In a post-op session the therapist works around:
- Incisions and drains. Strokes are planned around them. If drains are still in, read can you have lymphatic drainage with surgical drains in.
- Compression garments. The American Society of Plastic Surgeons notes you will wear an elastic bandage, support bra or compression garment after liposuction to minimize swelling. Getting in and out of it, and checking how it fits, is part of the visit.
- Fibrosis. The National Lymphedema Network’s 2025 position paper describes MLD pressure as adapted to tissue texture, firmer for fibrotic tissue and lighter for softer tissue. After surgery, that tissue changes from week to week.
- Your surgeon’s plan. Treatment can begin as early as 24 hours after surgery at Renu, but only with your surgeon’s approval and when there is no infection or other contraindication.
Classic sessions assume none of this. They are built for an intact lymphatic system: light, rhythmic work for fluid retention, heavy legs, puffiness or simply calm.
How long you stay on post-op sessions
Most people move from post-op to classic over a few months, as the swelling resolves. A 2023 review in the Aesthetic Surgery Journal Open Forum describes general recommendations of manual lymphatic massage two to three times a week in the first three to four weeks of recovery, and notes that swelling after liposuction can take three to six months to resolve. It also reports that studies have found MLD, with other therapies, reduced swelling and fibrosis and provided pain relief after liposuction and lipoabdominoplasty. The ASPS timeline has swelling starting to subside around weeks four to five, with continued improvement from week six, and the NHS says the area can take up to six months to settle completely.
At Renu, post-op care is planned in two stages: gentle swelling reduction first, then fibrosis management, typically starting at two to three sessions a week for the first two weeks and easing to weekly as swelling settles. Our answer on how many sessions you need after lipo goes into the numbers. If you are already months past surgery, it is not necessarily too late to start.
Prices and booking at Renu
The two ladders sit side by side so you can see the difference.
| Length | Classic lymphatic drainage | Post-op lymphatic drainage |
|---|---|---|
| 30 minutes | $95 | $105 |
| 45 minutes | $105 | $115 |
| 60 minutes | $130 | $140 |
| 90 minutes | Not offered | $210, for two or more procedures in one surgery |
Pre-op sessions are booked as classic lymphatic drainage, 60 minutes, $130; we offer one to three before surgery. In Jane, the 60-minute post-op session can also be booked by procedure, such as abdominoplasty, 360 lipo and mommy makeover, breast surgery, or face and neck. Post-op work is delivered by our Certified Lymphedema Therapist and lymphatic-trained RMTs, and cosmetic-surgery recovery is delivered through BodyTox by Renu in the same clinic. Compression garment fitting and ordering are available for every kind of surgery. See pre and post-op lymphatic drainage for the full service, or lymphatic drainage massage for classic sessions.
When to pause either session and call your surgeon
The same contraindications pause both kinds of session. Cleveland Clinic lists blood clots, deep vein thrombosis, cellulitis, fever, infection, heart disease, kidney failure and stroke as reasons to avoid lymphatic drainage. After surgery, call your surgeon rather than your therapist if you notice:
- Throbbing pain or swelling in one leg, usually the calf or thigh, which the NHS lists as signs of a blood clot
- Shortness of breath or chest pain (call 911)
- A fever, or an incision that looks infected
- Severe pain or any unexpected symptom, which the NHS advises reporting to your surgeon straight away after liposuction
Tell your therapist about every medication you take, and mention blood thinners in particular, since pressure and timing may need to change.
If you are still unsure
Choose post-op when in doubt during the first months after surgery; it is the session designed for healing tissue. If you are booking for general well-being with no surgery in your history, choose classic, and see post-op lymphatic drainage vs regular massage if you are wondering why deeper massage is not the right tool after surgery. You can also call the clinic, describe your procedure and date, and we will book the right one.
Questions people ask
I had surgery a year ago. Is that post-op or classic?
Tell us when you book. If the area is still firm, uneven or swells by the end of the day, a post-op session lets the therapist work on that tissue. If it feels settled and you simply enjoy lymphatic work, classic is fine.
Can I book a post-op session for a non-cosmetic surgery?
Yes. Post-op lymphatic drainage at Renu covers orthopedic surgeries such as knee, hip and shoulder procedures, oncology surgery and reconstructive surgery, as well as cosmetic procedures.
What if I had two procedures at once?
Book the 90-minute multiple surgeries session ($210), which is set aside for two or more procedures done in one surgery. It gives the therapist time to work on each area properly.
Is pre-op lymphatic drainage worth it?
Many people find it useful as a way to meet the therapist, learn what sessions feel like and prepare the tissue before surgery. Renu offers one to three pre-op sessions; ask your surgeon whether they suit your procedure.
