Massage for back pain after shovelling snow in Calgary: when to book, what guidelines say about massage and heat, and which symptoms need a doctor first.

Massage is a reasonable choice for a low back that has seized up after shovelling, once you are sure it is a muscle strain and not something more serious. The American College of Physicians lists superficial heat, massage, acupuncture and spinal manipulation as first options for new low back pain, because most of it improves within a few weeks whatever you do. Chest pain, light-headedness, or numbness or weakness in both legs while or after shovelling are emergencies, not reasons to book a massage.

Massage can help a low back that tightened up after clearing snow, and it is one of the options doctors’ guidelines suggest trying first. The American College of Physicians recommends starting new low back pain with non-drug care such as superficial heat, massage, acupuncture or spinal manipulation, partly because most of these backs settle within a few weeks regardless. Before you book, though, make sure the problem is your back and not your heart: shovelling is hard cardiac work, and chest symptoms come first. Why shovelling snow hurts so many backs

Shovelling combines lifting, twisting and throwing with a loaded blade held out in front of you, often first thing in the morning. A large American study counted an estimated 195,100 people treated in emergency departments for snow shovel incidents over 17 winters. Lower back injuries were about a third of those cases (34.3 per cent), and the most common mechanism was acute musculoskeletal exertion, which made up more than half. Slips and falls came next.

There is a local reason people rush it, too. Calgary’s Street Bylaw requires property owners and occupants to clear the sidewalk beside their property within 24 hours after a snowfall. That deadline, a heavy snowfall, and a commute to get to often add up to shovelling fast, early and cold.

Check these warning signs before you book a massage

Shovelling raises your heart rate quickly. The American Heart Association reports that after only two minutes of shovelling, study participants’ heart rates rose above 85 per cent of their maximum, a level similar to an intense treadmill test. In the American emergency department study, cardiac visits were 6.7 per cent of cases but included every one of the 1,647 deaths.

Call 911 if, during or after shovelling, you have:

  • chest pain or pressure;
  • light-headedness;
  • palpitations or an irregular heartbeat.

Get emergency care for back pain with:

  • numbness, tingling, pain or weakness in both legs;
  • loss of feeling around the genitals or anus;
  • new trouble passing urine, or loss of bladder or bowel control;
  • back pain after a hard fall, such as on ice.

See a doctor soon if you have back pain with fever or chills, pain that is severe and getting worse quickly, pain that shoots down one leg, or pain that is not easing after a few weeks. A massage therapist does not diagnose these problems, and we would rather you were seen first.

What massage can do for a shovelling strain

For a plain muscle strain, massage is a reasonable, low-risk way to ease pain and guarding while your back recovers. The ACP guideline rated the evidence for massage in new low back pain as low quality, so it is honest to say it helps some people feel better sooner rather than that it speeds healing. Superficial heat had moderate-quality evidence, which is why we often suggest pairing the two.

Between appointments: what helps a strained back at home

Keep moving. The NHS advises staying as active as you can and carrying on with daily activities rather than spending long stretches in bed. Other steps from its advice:

  • heat packs for muscle spasm and stiffness, or an ice pack wrapped in a towel for pain;
  • gentle exercises and stretches that do not make the pain sharply worse;
  • anti-inflammatory painkillers such as ibuprofen if they are suitable for you; ask a pharmacist if unsure.

Shovel the next snowfall with less strain

The American Heart Association’s advice protects both your heart and your back:

  • Push the snow rather than lifting and throwing it.
  • Take frequent short breaks, and do not shovel right after a big meal or after drinking alcohol.
  • Stop and call for help if you feel any of the warning signs above.
  • Do not shovel at all if you have heart disease, a past heart attack, bypass surgery or angioplasty, or major risk factors such as high blood pressure, diabetes, smoking or an inactive lifestyle. Ask a neighbour or hire help.

The shovel matters a little as well. In a small laboratory study of eight people lifting snow-like loads, a bent-shaft shovel lowered the load on the joint between the lowest lumbar vertebra and the sacrum by 16.5 per cent and reduced forward bending by 13 per cent compared with a straight shaft.

Booking a massage after a snowfall

Massage is booked online through Jane or by phone, seven days a week: 8:00 am to 8:30 pm on weekdays and 10:00 am to 5:00 pm on weekends, so an evening appointment after a morning of shovelling is usually workable. When you book, mention that it is a new back strain and how it happened, so your therapist can plan the session around it.

Questions people ask

How soon after hurting my back shovelling can I get a massage?

Once you have ruled out the warning signs and can get on and off a table, there is no rule that says you must wait. Many people book within the first few days, when the muscles are guarding hardest. If the back is too sore to lie face down, tell us and the therapist can treat you on your side.

Should I put heat or ice on my back after shovelling?

The American College of Physicians found moderate-quality evidence for superficial heat in new low back pain, more than for any other non-drug option. The NHS suggests heat packs for spasm and stiffness and wrapped ice packs for pain and swelling. Use whichever feels better, and never put either straight on bare skin.

Is it a muscle strain or a slipped disc?

A massage therapist cannot diagnose a disc problem. Pain that shoots down one leg, or numbness and weakness in a leg, deserves a doctor's assessment, and symptoms in both legs or around the saddle area are an emergency. Pain that stays in the back and eases with gentle movement is more typical of a strained muscle.

In one small laboratory study of eight people, a bent-shaft shovel reduced the load on the lowest joint of the spine by about 16 per cent and forward bending by 13 per cent compared with a straight shaft. It is a modest change from a small study, but pushing snow instead of lifting it does even more to spare your back.
