Massage for a thigh contusion is something to delay, not rush: deep work on a fresh charley horse is listed as a possible contributor to myositis ossificans, where bone forms inside the bruised muscle. Early care is about protecting the muscle and getting your knee bending again, and massage fits later, around the injury rather than on it, once your knee bend has returned. Severe, worsening pain with a tight, swollen thigh is an emergency.
Your knee bend tells you how bad it is
A thigh contusion, also called a charley horse, dead leg or cork, is bleeding and damage inside the quadriceps from a direct blow. The most useful measure of its severity is not the size of the bruise but how far you can bend your knee.
The classic grading comes from a three-year study of 117 quadriceps contusions in West Point cadets, measured 12 to 24 hours after the injury:

A blow that leaves you barely able to bend the knee is a significant injury, even if the skin looks only mildly bruised.
The first 24 hours: knee bent, not massaged
Early care aims to keep the knee bending and limit bleeding into the muscle. One approach has particularly quick results. At the United States Naval Academy, 47 midshipmen with quadriceps contusions had the knee gently bent to 120 degrees within minutes of the injury and held there for 24 hours. After that they did pain-free quadriceps stretching several times a day and pain-free strengthening as soon as they could. They returned to full activity in an average of 3.5 days (range 2 to 5), and only one of the first 23 X-rayed developed bone in the muscle.
That series had no comparison group, as a later review points out, so it shows what is possible rather than proving the method beats others. Its authors also gave a clear return-to-play marker: getting back 120 degrees or more of knee bend.
For your first couple of days, NHS advice for muscle injuries applies: protect and rest the leg, use ice, and avoid heat, alcohol and massage to help prevent swelling. A sports physician or physiotherapist can show you how to hold the knee bent safely.
Why we do not knead a fresh charley horse
The specific risk with a thigh contusion is myositis ossificans: bone forming inside the bruised muscle. In the West Point cadets it developed in 9 percent, and five factors were linked with it:
- knee bend of less than 120 degrees
- the injury happening during football
- a previous quadriceps injury
- treatment delayed by more than three days
- fluid in the knee joint on the same side
A review of muscle injuries in Sports Health adds that the condition is most common in the quadriceps and the upper arm, that it is more frequent in contact sports where protection is limited, and that people with bleeding disorders may be at higher risk. It also lists early vigorous massage and excessive mobilisation as clinical factors that may contribute.
That is why trying to “work the knot out” of a fresh dead leg is a poor idea. Here our role is to adapt the timing: deliberately later, and gentler on the injured spot.
When massage fits
Once the acute phase has passed, the bruise is no longer hot or swelling, and your knee bend is coming back, massage can help the rest of your body cope with the injury:
- Around, not on, the bruise. The hip, glutes, hamstrings, calf and low back often tighten from limping. Work on the contused area itself stays light, and only once it is no longer tender.
- The other leg. It has been doing extra work.
- No forcing the bend. Stretching stays pain-free and follows your physiotherapist’s plan, not the massage table.

When to see a doctor
Call 911 or go to an emergency department if, after a blow to the thigh, you develop sudden, severe pain, especially pain that gets worse when the muscle is stretched, with swelling, tightness, numbness, weakness or pins and needles. The NHS describes this as acute compartment syndrome, where pressure inside the muscle restricts blood flow; it needs surgery as soon as possible.
See a doctor soon if:
- your knee bend is not improving after a few days, or is getting worse
- the swelling or bruising is large or still increasing
- walking more than a few steps on the leg is not possible
- you take blood thinners or have a bleeding disorder
- a firm lump develops or persists in the muscle weeks later
That last sign can be myositis ossificans. It is benign and usually settles over time, but a review in the Journal of the American Academy of Orthopaedic Surgeons notes it can look like more serious masses on imaging and may need several types of scan. If surgery is ever needed, waiting up to a year for the bone to mature may be advised to lower the chance of recurrence.
Questions people ask
Is a charley horse a cramp or a bruise?
People use the name for both. A cramp is a muscle painfully tightening on its own; a contusion is a bruise inside the muscle from a direct blow, and it leaves the thigh sore and stiff for days. This page is about the bruise.
Should I put heat on a dead leg?
Not in the first couple of days. NHS advice for muscle injuries is to avoid heat, alcohol and massage early on to help prevent swelling, and to use ice and protection instead.
Why is my thigh still hard weeks after the hit?
A firm lump that persists or grows weeks after the hit can be myositis ossificans, which is benign but can look like other masses on imaging, so it needs a doctor and possibly several types of scan to confirm.
How do I know when I can play again?
Getting back at least 120 degrees of knee bend is the marker used for a safe return after a quadriceps contusion. A sports physician or physiotherapist can measure that and check your strength.
