Lymphatic drainage after Belkyra (Kybella) in Calgary: why the chin swells so much, gentle neck drainage once your injector agrees, and the signs to report.

Big swelling under the chin after Belkyra, the Canadian name for Kybella, is the norm: it affected 87 percent of treated patients in the trials and usually lasted around nine to ten days. Gentle lymphatic drainage of the neck and collarbones, with no pressure on the firm treated area, is a reasonable comfort option once your injector agrees. It does not change how much fat the injections remove, and an uneven smile, trouble swallowing or a hot, red, worsening area go to your injector first.

Why the swelling is so big

Belkyra is the brand name in Canada for deoxycholic acid injection, the same drug sold in the United States as Kybella. It is injected into the fat under the chin, where it breaks down fat cell membranes. The American Society of Plastic Surgeons (ASPS) describes it as chemically reducing the number of fat cells around the injection site.

That breakdown is an injury, and the body responds with inflammation. The Canadian product monograph, which reports the side effects seen in the two large Phase 3 trials (515 people treated with Belkyra, 504 with placebo), shows how common the after-effects are:

Bruising happened almost as often with placebo, which tells you it comes mostly from the needles. Swelling, numbness and firmness come from the drug.

How long does it last? In a European Phase III trial published in the British Journal of Dermatology, swelling lasted a median of nine to ten days on the two Belkyra doses, compared with three days on placebo, and numbness and firmness generally settled within 17 to 25 days. Most side effects resolved within the four-week gap between sessions.

Where lymphatic drainage fits

Lymphatic drainage after Belkyra is a comfort option, not part of the treatment. No study we have found tests it after deoxycholic acid, so we cannot tell you it shortens the swelling or improves your result. What we can say is how the work would be done safely, if your injector is comfortable with it.

Cleveland Clinic describes a lymphatic drainage session as usually starting with light work over the areas that house lymph nodes, including the neck, before the swollen area itself.   1. Collarbones first. Light, slow strokes just above the collarbones.
  2. Sides of the neck. Feather-light skin stretching down toward the collarbones, avoiding the front of the throat.
  3. The edges of the treated zone. Only if comfortable, and only at the margins, moving fluid down and out.
  4. The injected area itself. Left alone: no pressure on firm patches, nodules or tender spots.

Pressure is very light throughout. Numbness in two-thirds of people means you may not feel when pressure is too much, so we keep it lighter than you might expect. Our page on whether it is safe to massage the front of the neck covers why the throat itself is avoided.

Timing around your injection schedule

Belkyra is usually given as a series. The monograph allows up to six treatments, spaced no less than one month apart, and the ASPS says most patients have at least two treatments four to six weeks apart, with fat reduction typically visible by six to eight weeks after a treatment.

A sensible rhythm for lymphatic work:

  • First two or three days: let the acute pain settle. In the trial, pain lasted a median of one day.
  • Days three to 14: the window when swelling is at its peak, and when a gentle session is most likely to feel helpful.
  • Before your next injection: let your injector see the area untouched. The monograph says Belkyra should not be given into skin that is still inflamed or hardened, so they need an honest look.

When to call your injector first

The monograph lists a few effects that are not ordinary swelling. Contact the clinic that did your injections, and skip your lymphatic session, if you notice:

  • An uneven smile or weakness of the lower lip. Injury to the marginal mandibular nerve, which can show as an asymmetric smile, was reported in 4 percent of treated patients and lasted from 1 to 298 days (median 44).
  • Difficulty swallowing. It occurred in 2 percent of trial patients, lasting from 1 to 81 days (median three). Severe difficulty swallowing or breathing needs urgent care.
  • Signs of infection. Post-market reports include injection site infections, some with cellulitis or abscess that needed medical treatment. Watch for increasing heat, redness, pain, pus or fever.
  • Skin breaking down. Ulceration and tissue damage at the injection site have been reported.
  • Hair loss in the treated area, which the monograph also lists.

None of these are treated with massage, and some need attention quickly.

Questions people ask

Why does my chin look bigger after Belkyra than before?

Swelling. The American Society of Plastic Surgeons notes that it can temporarily make the area look fuller before the improvement shows. In the trials the swelling typically lasted about nine to ten days, and fat reduction is usually visible six to eight weeks after a treatment.

Can I massage the hard lumps under my chin?

Leave them alone. Firmness and small nodules are listed side effects that usually soften with time, and in one trial numbness and firmness generally resolved within 17 to 25 days. Rubbing tender, freshly injected tissue can make it sorer; mention lumps that persist to your injector.

How many Belkyra sessions will I need?

It varies. The Canadian product monograph allows up to six treatments spaced no less than one month apart, and the American Society of Plastic Surgeons says most patients have at least two. Lymphatic drainage does not reduce the number you need.

Should I book lymphatic drainage right before my next injection?

Talk to your injector about timing. The monograph says Belkyra should not be given where the skin is still inflamed or hardened, so your injector will want to see the area as it really is. Many people book drainage in the first two weeks after a session, when swelling is at its worst.
